Support Coordination

Support Coordination helps you understand, implement, and make the most of your NDIS plan. Our experienced coordinators work alongside you to connect you with the right services and supports — so you can focus on living your life.

We take the complexity out of the NDIS. From navigating your plan to connecting you with local services, allied health providers, and community groups, we're here to support you in building the skills and confidence to manage your plan effectively.

What's Included:

Understanding and implementing your NDIS plan
Connecting you with local service providers and community supports
Coordinating appointments, therapy, and allied health services
Assisting with communication between you and service providers
Preparing for plan reviews and goal setting
Supporting you to build capacity and solve challenges as they arise
Regular check-ins and progress monitoring

Making Your Plan Work For You

Many NDIS participants don't fully utilise their plan funding — often because navigating the system is complex and time-consuming. That's exactly where we come in.

Our support coordinators are experienced in working with participants across a range of disability types and age groups, from children and young adults to older Australians.

Capacity Building Support Coordination is funded under your plan's Capacity Building budget — at no out-of-pocket cost to you.

Supported Independent Living

Supported Independent Living (SIL) provides in-home support to help you live as independently as possible in your own home or shared accommodation. Our trained support workers are there to assist with daily living tasks while respecting your independence, choices, and privacy.

We work closely with participants, families, and other providers to create a support environment that truly reflects the individual's goals, routines, and preferences.

What's Included:

Assistance with personal care and hygiene
Meal preparation and nutrition support
Household tasks and home maintenance assistance
Support with medication management
Community access and social participation
Overnight and 24/7 support options
Skills development for greater independence

Your Home, Your Rules

SIL is about more than just assistance — it's about building your confidence and skills so you can live the life you want, in your own home.

All SaS support workers are fully background-checked, trained in disability support, and matched thoughtfully to participants to ensure compatibility and comfort.

Core Supports SIL is funded under your NDIS Home and Living funding. Our team can help assess your eligibility and apply for appropriate funding levels.
